Today’s freebie is a slick designed contact form in PSD format. Elements of this contact form are transparent so it looks good on various backgrounds. I added a subtle pattern as an overlayer for a more modern look. It fits great with a greyscale image as a background like seen in a preview. Image was bought on PhotoDune for preview purpose only and is not available in the source files.
